Dead mouse, live roaches and sewer water closed Philly eateries

A dead mouse in a trap, toilets that won't flush, problems with hot water, and expired baby formula highlight the violations in the latest Clean Plates report.

Between Jan. 16 and Jan. 31, Philadelphia Department of Public Health inspectors temporarily closed 24 restaurants, markets, and stores.

The inspectors visit nearly 1,000 eateries and food retailers every two weeks. Each inspection is generally regarded as a snapshot in time, and not necessarily a reflection of day-to-day conditions. Most violations were corrected immediately in the presence of an inspector.

Pike's Seafood
1940 E Washington Ln.
13 violations, 1 serious
A dead mouse was found on a trap in the rear storage area; mouse droppings were found in the back storage area; facility lacked an adequate amount of drainboards to accommodate all soiled and cleaned items that accumulated during hours of operation; floor surfaces were not in good repair; there were uneven cement surfaces; holes, gaps, and unsealed pipe penetrations found in the ceilings and walls in the food prep and ware washing areas; heavy ice accumulation found in the walk-in freezer unit.
Due to imminent health hazards observed during this inspection, the establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected Jan. 29.

El Nino Grocery Inc.

Standing sewage water was found in the basement area; nine cans of expired Nestle Nido formula found on retail shelving; mouse droppings found on the shelving throughout the food prep, retail, and basement areas; the front door did not protect against the entry of insects, rodents, and other animals; the slicer had an accumulation of food residue on the food contact surfaces and was not being washed, rinsed, and sanitized at least every four hours; there were opened deli meats and cheeses located in the display case, that were not marked; thermometers for ensuring proper temperatures of food were not available or readily accessible; there were unapproved griddle, toaster oven, fryer, and drying racks in the facility.
Due to imminent health hazards observed during this inspection, the establishment was issued a Cease Operations Order and must discontinue food operations immediately. The establishment could not operate for a minimum of 48 hours. Inspected Jan 29.

Palm oil was found in containers that were marked as vegetable oil or unlabeled; there were damaged bags of lamb found in the freezer; the sanitizing system was not functioning; there was a clogged floor drain and indirect drain in the rear; cold water was not available at the hand wash sink in the ware washing area; an employee was seen eating or tasting food from the vegetable scale and prep area; employees were donning single use gloves without a prior hand wash; the oven thermometer was found in the freezer; mouse feces was found in the rear area; the walk-in freezer was not sealed to the wall.

The slicer had an accumulation of food residue on the food contact surfaces and was not being washed, rinsed, and sanitized at least every four hours; there was debris in the retail area behind ATM machine; a hole was found in the ceiling of the retail area; the lightbulbs in the retail area ceiling at the service window were not shielded from breakage; mouse droppings were found in the ware washing area; the garbage disposal was installed improperly; food debris was found inside bottom of refrigeration units; the dumpster was found open.

Ground sausage prepared from food ingredients was held at improper temperature; food including cheese, lamb, pork and shellfish were held at improper temperatures; hot water was not at required temperature; employee bathroom needed cleaning, the customer bathroom vents needed cleaning; there was an accumulation of dust and debris under equipment in outside bar area; an employee was not wearing a beard restraint.
This inspection has revealed that the establishment is not in satisfactory compliance. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection. Letter of discontinued food preparation of sous vide must be provided to Office of Food Protection. Inspected Jan 23.

Several raw animal foods were stored above ready-to-eat foods in units; mouse droppings were found on the food prep table, the rice cooker, utensils, and along shelves where food containers are stored; chicken was not held at the proper temperature; chicken was not being thawed using an approved method; a cellphone was found on a food contact surface; the wiping cloths were dirty; the hood filters had an accumulation of static dust and grease; a general cleaning was needed along the wok station in between and behind all equipment due to heavy grease/crud accumulation.

Soap and paper towels were not present inside toilet room.

A live roach was seen crawling on the wall between service area and food prep area; there were fresh mouse droppings on lower level of steam table and on shelves in service area; a baking sheet was found in the hand wash sink in the food prep area; food including chicken, and meat patties were held at improper temperatures; unwrapped cheese was stored in a cardboard box, sugar at coffee station was uncovered, pizza dough was stored in black garbage bag, lettuce and a cut tomato were left uncovered in the walk-in cooler; dust and mouse droppings observed on storage shelf beneath steam table and shelves in service area; there was dust and debris in the inoperable cold-holding unit where candy was stored, there was debris and broken glass in the walk-in beverage cooler.

The Food Safety Certificate expired on May, 1, 2017. The person in charge was instructed to renew her Food Safety Certificate or get somebody with valid Food Safety Certificate and then call for reopen inspection.

There was a heavy accumulation of rodent feces found on all tiers of shelving and around floor perimeter in the retail area, as well as on floor, under shelves, and in the walk-in cooler; packets of orange rodenticide were observed on the basement steps. Rodent bait shall be contained in a covered, tamper-resistant bait station; the restroom hand wash sink was being supported by wooden planks; there was evidence of smoking in the rear storage area that includes ashes inside a styrofoam cup, lighter, matches, cigarette, and an odor; there was clutter in the basement that would prevent adequate floor cleaning and can provide harborage areas; the ice cream freezers were in need of cleaning due to heavy ice buildup.
